Step 1: Select a full length session video and upload it to your clinical evaluation folder shared with you by your direct supervisor. Make sure to select the correct Month and Year.

Step 2: Use the clinical evaluation form while watching your video to self rate on the left side of the clinical evaluation form. This form is fixable so no need to print.

Step 3: Upload your clinical evaluation form to you clinical evaluation folder.

Step 4: Let your supervisor know that you have completed steps 1-3.

Once you submit your clinical evaluation form o your supervisor they will review the video and also rate your video.

You will sit down together and go over ares of improvement and area of continued growth.

Rest assured that all feedback is strengths based and constructive with the sole goal of helping you grow as a therapist and hone the skills needed to prepare for licensure.
